Family Promise of Warren County receives $20,000 grant from Atlantic Health Hackettstown Medical Center Community Advisory Board
PHILLIPSBURG, N.J. — Family Promise of Warren County has received a $20,000 grant from the Atlantic Health Hackettstown Medical Center Community Advisory Board to support a program aimed at helping medically vulnerable individuals and families experiencing housing instability.
The funding will support Home Rx: Housing for Health, a program designed to address the intersection of homelessness and health by providing case management and connecting participants with housing, health care and other essential services.
“This grant allows us to continue serving the most vulnerable in our community through intensive, person-focused case management that treats everyone as an individual to improve overall health and well-being,” said Rachel Opdyke, Executive Director of Family Promise of Warren County. “We are so grateful for Hackettstown Medical Center’s continued partnership and support in addressing the needs of the community we all serve.”
Over the next 12 months, the Home Rx: Housing for Health program is expected to serve 150 medically vulnerable individuals. The program’s goals include helping 80% of participants obtain or maintain stable housing and connecting at least 75% with primary or behavioral health care.
Participants will receive ongoing case management and coordinated support intended to bridge gaps between housing, health care and other essential services.
The Hackettstown Medical Center Community Advisory Board identified priority health needs in the area using data analysis and a community stakeholder survey. The board awarded grants to 13 organizations proposing initiatives addressing one or more of those health needs during the summer 2026 grant period.

Family Promise of Warren County is an independent nonprofit organization that has served families experiencing homelessness since July 2008.
