
Hunterdon Health Foundation receives $22K in grants to support food and health access programs
HUNTERDON COUNTY, N.J. — The Hunterdon Health Foundation has received two grants totaling $22,000 to support critical community health initiatives in the greater Hunterdon County area, officials announced.
A $10,000 grant from the TD Charitable Foundation will fund the Healthy Harvest Rx program, which combats food insecurity and chronic health conditions like diabetes and obesity. The program provides fresh produce, nutrition education, and healthy living strategies to approximately 85 patients annually. In partnership with America’s Grow-a-Row and local supermarkets, Healthy Harvest Rx ensures year-round access to healthy food options. Outcomes are tracked through participant surveys and health metrics, including improved blood sugar levels and weight loss.
A $12,000 grant from Solventum will aid the Community Care Liaison program, which connects underserved residents with healthcare services. Through a team of bilingual Community Health Workers, the initiative addresses barriers such as transportation, cost, and language access. The program aims to attend 50 community events, reach 7,000 individuals, and help 400 patients annually with tasks like scheduling appointments and accessing resources.
